Position Summary

Provides personal care services under the direction of the Registered Nurse or Therapist. The Home Health Aide is assigned to specific clients by the Registered Nurse or other appropriate professional and performs services necessary to maintain their personal comfort.

Reports To: Clinical Supervisor / Director of Clinical Services

Qualifications

• Successful completion of a formal certification training program and/or a written skills test and competency evaluation.
• At least 18 years of age.
• Minimum of 6 months work experience in a supervised setting, preferably a healthcare facility.
• Ability to read, write, and follow a written Plan of Care.
• Good verbal, written, and interpersonal skills.
• Licensed driver with insured automobile, in good repair, and a good driving record.

Essential Functions / Areas of Accountability

• Performs simple procedures as an extension of therapy services under supervision.
• Range of motion exercises.
• Assistance in ambulation or exercises.
• Performs personal care activities such as bathing, shampooing, skin/nail care, oral hygiene, shaving, dressing.
• Performs household services essential to health care at home, including meal preparation/feeding, laundry, light housekeeping.
• Assists in medication administration as directed by the Registered Nurse.
• Reports changes in the client's condition to the nurse or therapist, following reporting guidelines.
• Documents care provided and completes required forms, submitting documentation weekly.
• Promotes safety and infection control, reporting unsafe situations.
• Demonstrates safe use of equipment and reports educational needs.
• Communicates effectively with the interdisciplinary team.
• Maintains confidentiality.
• Attends in-service programs and identifies learning needs.
• Performs other duties as necessary.
