Guardian Angel Senior Services, Inc. is seeking a compassionate Registered Nurse (RN) to join our Home Care team in MA. The role focuses on helping seniors stay safe and comfortable at home while cultivating meaningful relationships with clients.

As a Home Care RN, you will assess needs, develop care plans, supervise aides, and coordinate with physicians, families, and the care team. You will document visits, ensure regulatory compliance, and deliver high-quality, patient-centered care.

Qualifications

• Current RN license in good standing.
• Home health, hospice, or community health experience preferred.
• Strong assessment and critical thinking skills.
• Excellent communication and organizational abilities.
• Ability to work independently while collaborating with a team.
• Reliable transportation and valid driver’s license.

Responsibilities

• Complete initial client assessments and reassessments.
• Develop and update individualized care plans.
• Supervise Home Health Aides, CNAs, and caregivers.
• Conduct routine supervisory and quality assurance visits.
• Provide client and caregiver education.
• Coordinate care with physicians, families, and interdisciplinary team members.
• Ensure compliance with state regulations and agency policies.
• Accurately document visits and clinical notes.
