Job summary

Guardian Angel Senior Services in Pittsfield, MA is seeking a compassionate Registered Nurse (RN) to join our Home Care team. You’ll work independently, build meaningful relationships with clients, and help seniors stay safe and comfortable at home.

As an RN Home Care clinician, you will assess clients, develop and update care plans, supervise aides, educate families, coordinate with physicians, and ensure compliance with state and agency policies.

Qualifications

• Current RN license in Massachusetts.
• Experience in home health, hospice, or community health preferred.
• Strong assessment and critical thinking skills.
• Ability to work independently while collaborating with a team.

Responsibilities

• Complete initial client assessments and reassessments.
• Develop and update individualized care plans.
• Supervise Home Health Aides and CNAs.
• Conduct supervisory and quality assurance visits.
• Provide client and caregiver education.
• Coordinate care with physicians and families.
• Document visits and clinical notes accurately.

What You'll Do

As a Home Care RN, you will play a vital role in ensuring our clients receive exceptional care by:

• Completing initial client assessments and reassessments
• Developing and updating individualized care plans
• Supervising Home Health Aides, CNAs, and caregivers
• Conducting routine supervisory and quality assurance visits
• Providing client and caregiver education
• Coordinating care with physicians, families, and interdisciplinary team members
• Ensuring compliance with state regulations and agency policies
• Accurately documenting visits and clinical notes

What We're Looking For

• Current Registered Nurse (RN) license
• Home care, hospice, or community health experience preferred
• Strong assessment and critical thinking skills
• Excellent communication and organizational abilities
• Ability to work independently while collaborating with a supportive team
• Reliable transportation and valid driver's license
• Proficiency with electronic documentation
