Job summary

Guardian Angel Senior Services is seeking a compassionate Registered Nurse (RN) to join our Home Care team. You will work independently to assess clients, develop care plans, and supervise aides to ensure seniors remain safe and comfortable at home.

Ideal candidates hold a current RN license, have strong assessment and communication skills, and can coordinate with families and physicians, while adhering to regulations. Flexible scheduling and competitive pay are offered.

Qualifications

• Must hold a current RN license.
• Home care, hospice, or community health experience is preferred.
• Strong assessment, critical thinking, and communication skills.
• Excellent organizational abilities and reliability.
• Ability to work independently while collaborating with a team.
• Valid driver's license and reliable transportation.
• Proficiency with electronic documentation systems.

Responsibilities

• Complete initial client assessments and reassessments.
• Develop and update individualized care plans.
• Supervise Home Health Aides and CNAs.
• Conduct supervisory and quality assurance visits.
• Provide client and caregiver education.
• Coordinate care with physicians, families, and interdisciplinary team.
• Ensure compliance with state regulations and agency policies.

• Conducting routine supervisory and quality assurance visits
• Providing client and caregiver education
• Coordinating care with physicians, families, and interdisciplinary team members
• Ensuring compliance with state regulations and agency policies
• Accurately documenting visits and clinical notes

What We're Looking For

• Current Registered Nurse (RN) license
