Job summary

Central Vermont Home Health & Hospice is seeking a Nurse Practitioner to deliver compassionate home health and hospice care. You will conduct visits, assess patients, and collaborate with certifying physicians to optimize outcomes.

You will support the care team, participate in care planning, and help advance quality of life and comfort for patients in our central Vermont community.

Qualifications

• Graduate of an approved Nurse Practitioner curriculum.
• Active APRN or NP license required.
• Minimum of two years clinical nursing experience.
• Valid driver’s license.

Responsibilities

• Conducts face-to-face encounters and documents findings for eligible patients.
• Performs well-patient checks and symptom management visits.
• Provides physician support and coordinates with IDT members.
• Develops Plan of Care for assigned patients with the care team.
• Collaborates with Hospice Clinical Manager and Medical Director to improve care strategies.
• Acts as resource for Hospice staff to ensure continuity of care.
• Attends IDT meetings and supports medical needs of hospice patients as needed.

Responsibilities

• Conducts face-to-face encounters for patients in their third or subsequent benefit period, documenting and sharing findings with certifying physician promptly.
• Conducts well-patient checks.
• Conducts symptom management visits.
• Provides physician support.
• Coordinates and collaborates with IDT members, community physicians, facility staff, and others as needed.
• Participates in developing Plan of Care for assigned patients.
• Assists in enhancing care strategies with Hospice Clinical Manager and Medical Director.
• Acts as a resource for Hospice staff to ensure continuity and improve patient care quality.
• Attends IDT meetings.
• Provides backup support to Hospice Medical Director, covering medical needs of hospice patients as required
