Job summary

First Day Homecare, LLC in Kalamazoo, MI is seeking a skilled Licensed Practical Nurse (LPN) to perform nursing functions under the direction of a medical practitioner. You will work under the Clinical Supervisor and provide 1:1 care in clients’ homes.

Responsibilities include administering medications, following care plans, and coordinating with the client’s care team to ensure safety. Weekend and shift variability are possible in a home-care setting.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current LPN license to practice in the state of operation.
• Current CPR certification.
• Demonstrated written, verbal, and interpersonal communication skills.
• Licensed driver with automobile insured in accordance with state requirements.

Responsibilities

• Provides professional nursing care as defined by the Nurse Practice Act.
• Performs care per medical orders and client care plans.
• Maintains and documents orders in the medical record per agency procedures.
• Clinical duties include medication administration, client teaching, emergency intervention, and pediatric care such as tracheostomy care and ventilator management.
• Collaborates with care team and promotes safety for clients and coworkers.
• Performs duties in compliance with agency policies and standards.
• Participates in care coordination and agency meetings as requested.
• Performs other related duties as needed.
• Protects client privacy and confidentiality.

Schedule / Expected Work Hours

• Kalamazoo, MI: 8-hour weekend day shifts.
• Kalamazoo, MI: 8-12-hour night shifts
• Kalamazoo, MI: 8-hour afternoon shifts.

Job Summary

Performs skilled nursing functions in accordance with federal, state, and local laws and within the guidelines of professional standards and the Nurse Practice Act. All nursing services provided will be under the direction of medical practitioner (MD, DO, NP, PA, etc.) orders.

Reports To: Clinical Supervisor

Provides Supervision To: NA – No supervision responsibilities

Responsibilities

• Provides professional nursing care as defined in the state Nurse Practice Act.
• Provides care in accordance with medical practitioner orders and the client’s care plan.
• Maintains orders for clients and documents all changes in orders in the medical record following Agency procedures.
• The scope of clinical practice includes personal care services, monitoring and reporting findings, medication administration, client teaching, emergency intervention, and specialized pediatric services such as tracheostomy care, ventilator management, and tube feedings.
• Works collaboratively with other members of the client’s care team.
• Promotes personal safety and a safe environment for clients and coworkers.
• Performs job in compliance with Agency policies, procedures, and applicable payor and accreditation standards.
• Participates in the coordination of care and attends Agency meetings as requested.
• Performs other related duties and responsibilities as deemed necessary.
