First Day Homecare is seeking an experienced LPN to provide pediatric private duty nursing across clients in Ottawa County, Michigan. You will work under physician orders with a focus on tracheostomy care, ventilator support, and feeding tube management, while documenting care and following state regulations.

Reports to Clinical Supervisor and coordinates with the care team to ensure high-quality, family-centered services.

Qualifications

• Must hold a current LPN license in the operating state.
• CPR certification is required.
• Demonstrated strong written, verbal, and interpersonal communication skills.

Responsabilités

• Provide professional nursing care per the state Nurse Practice Act.
• Follow orders from medical practitioners and the client’s care plan.
• Maintain client orders and document changes in the medical record per agency procedures.
• Perform clinical tasks including personal care, monitoring, reporting, medication administration, client teaching, and emergency interventions; provide pediatric-specific care such as tracheostomy, ventilator management, and tube feedings.
• Collaborate with the client’s care team.
• Promote safety and a safe environment for clients and coworkers.
• Adhere to agency policies, procedures, payor, and accreditation standards.
• Participate in care coordination and agency meetings as requested.
• Perform other related duties as needed.

Schedule / Expected Work Hours:

• Seeking day and night shifts for clients throughout Ottawa County. Working with clients ages 0 and up with trachs, vents, and feeding tubes.

Job Summary

Performs skilled nursing functions in accordance with federal, state, and local laws and within the guidelines of professional standards and the Nurse Practice Act. All nursing services provided will be under the direction of medical practitioner (MD, DO, NP, PA, etc.) orders.

Reports To: Clinical Supervisor

Provides Supervision To: NA – No supervision responsibilities

Responsibilities

• Provides professional nursing care as defined in the state Nurse Practice Act.
• Provides care in accordance with medical practitioner orders and the client’s care plan.
• Maintains orders for clients and documents all changes in orders in the medical record following Agency procedures.
• The scope of clinical practice includes personal care services, monitoring and reporting findings, medication administration, client teaching, emergency intervention, and specialized pediatric services such as tracheostomy care, ventilator management, and tube feedings.
• Works collaboratively with other members of the client’s care team.
• Promotes personal safety and a safe environment for clients and coworkers.
• Performs job in compliance with Agency policies, procedures, and applicable payor and accreditation standards.
• Participates in the coordination of care and attends Agency meetings as requested.
• Performs other related duties and responsibilities as deemed necessary.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current license to practice as an LPN in the state of operation.
• Current CPR certification.
• Demonstrated written, verbal, and interpersonal communication skills.
• Licensed driver with automobile insured in accordance with state and/or Agency requirements.
