Job summary

Trinity Health in Mishawaka, IN seeks a part-time CNA to provide personal care and support for elderly participants at the St Joseph PACE facility. You will assist with activities of daily living, mobility, bathing, grooming and meal service, while promoting independence and safety in a compassionate, patient-centered environment.

Must hold an active Indiana CNA license, have at least one year of frail-elderly experience, a valid driver’s license, and strong written and verbal communication.

Employment Type: Part time Shift: Day Shift Description: CNA – Certified Nursing Assistant Shift: First Shift Status: Part Time Location: St Joesph PACE

Our facility ministers exclusively to the elderly through a coordinated care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community. This position will be housed primarily at the St Joesph PACE facility.

What you will do:

• Provide personal care assistance maintaining the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming
• Assist participants in the maintenance of physical activity, exercise, and walking
• Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance
• Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping

What you will need:

• Must be an active certified nurse assistant in the state where PACE Organization is located
• Minimum of one (1) year of documented experience working with a frail or elderly population strongly preferred
• Valid driver's license
• Superior written/verbal communication skills
• Must be customer focused, exercise independent judgment and flexibility to adapt to ongoing change
• Proficiency with Microsoft Office (MS Word, Excel, Power Point, etc.) and basic knowledge/experience with electronic mail and calendaring systems
• Position may require 10% local travel and use of personal vehicle
• Position requires ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting
• Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable
