Job summary

Trinity Health is seeking a CNA to provide personalized care for an elderly population at the Mishawaka/PACE facility. The role focuses on assisting with daily living activities and maintaining independence within a supportive, patient-centered environment.

You will help residents with mobility and activity, offer behavioral support, and assist with meals and daily tasks while adhering to care plans and safety guidelines. A sign-on bonus is offered along with competitive benefits.

Qualifications

• Active CNA license in the state where PACE Organization is located.
• Minimum 1 year experience with frail/elderly population preferred.
• Proficiency with MS Office and basic email/calendar skills.

Responsibilities

• Provide personal care to maintain participant independence in ADLs (toileting, bathing, eating, grooming).
• Assist with mobility, exercise, and walking.
• Provide behavioral supervision and nursing/medication reminders, dietary restrictions, and emotional support.

Incentives: $2500 Sign on Bonus AND 1st Day Benefits

Our facility ministers exclusively to the elderly through a coordinated care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community.

This position qualifies for a $2500 sign on bonus with a 2-year commitment. The 3 installments are paid out over the two-year commitment period.

This position will be housed primarily at the St Joesph PACE facility.

What you will do:

• Provide personal care assistance maintaining the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming

• Assist participants in the maintenance of physical activity, exercise, and walking

• Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance

• Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.

What you will need:

• Must be an active certified nurse assistant in the state where PACE Organization is located.

• Minimum of one (1) year of documented experience working with a frail or elderly population strongly preferred

• Valid driver's license

• Superior written/verbal communication skills

• Must be customer focused, exercise independent judgment and flexibility to adapt to ongoing change

• Proficiency with Microsoft Office (MS Word, Excel, Power Point, etc.) and basic knowledge/experience with electronic mail and calendaring systems

• Position may require 10% local travel and use of personal vehicle.

• Position requires ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
