Job summary

VNA Health Group's Children & Family Health Institute (CFHI) in Middlesex County, New Jersey seeks a Registered Nurse for the HomeVisitorRN role under the Nurse-Family Partnership program to support first-time mothers and families.

You will perform in-home visits, coordinate care, coach families on health and child development, and collaborate with a team; bilingual English/Spanish preferred; caseload up to 25; union position.

Qualifications

• Associate's degree in Nursing required.
• 2-3 years of relevant nursing experience; homevisiting or maternal/child health preferred.
• Proficiency in computer use, excellent verbal and written communication, strong organizational and interpersonal abilities.
• Bilingual English/Spanish preferred.
• Commitment to CFHI’s mission and ability to build trusting relationships with clients and colleagues.

Responsibilities

• Conduct in-home visits to provide nursing services aligned with the NFP model and Visit-to-Visit guidelines, caseload up to 25 clients.
• Assess and address the health and developmental needs of women and families.
• Offer counseling and instruction to help families achieve targeted health and life goals.
• Build and maintain strong community relationships for client referrals and resource connections.
• Attend weekly team meetings, case conferences, and clinical supervision sessions.
• Mentor and precept new hires and support student nurses during home visits as needed.
• Ensure compliance with infection control and safety policies.
• Maintain professionalism per organizational policies on absenteeism and appearance.

As a HomeVisitorRN, you will:

This position is covered by a collective bargaining agreement and is represented by a Union.

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $42.00 to USD $50.00.
