Job summary

Children & Family Health Institute (CFHI) at VNA Health Group seeks a compassionate Home Visitor RN to deliver nursing care under the Nurse-Family Partnership (NFP) program in Somerset County, New Jersey. Build rapport with first-time mothers, support healthy pregnancies, and promote positive child development.

The role supports CFHI's mission with in-home visits, counseling, and community referrals. Bilingual English/Spanish required; RN with 2–3 years of maternal/child health experience

Qualifications

• Associate's degree in Nursing is required.
• 2-3 years of nursing experience; home visiting or maternal/child health experience is preferred.
• Proficiency in computer use, excellent verbal and written communication, and strong organizational and interpersonal abilities.
• Bilingual English and Spanish-speaking is required.
• Commitment to CFHI's mission and the ability to build trusting relationships with clients and colleagues.

Responsibilities

• Conduct in-home visits to provide nursing services aligned with the NFP model and Visit-to-Visit guidelines, managing a caseload of up to 25 clients.
• Assess and address the health and developmental needs of women and their families.
• Offer counseling and instruction to help families achieve targeted health and life goals.
• Build and maintain strong community relationships to facilitate client referrals and resource connections.
• Attend weekly team meetings, case conferences, and clinical supervision sessions.
• Mentor and precept new hires and support student nurses during home visits as needed.
• Ensure compliance with agency infection control and safety policies.
• Maintain professionalism in adherence to organizational policies on absenteeism and appearance.

As a Home Visitor RN, you will:

This position is covered by a collective bargaining agreement and is represented by a Union.

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $42.00 to USD $50.00.
