Job summary

VNA Health Group's CFHI is hiring a Home Visitor RN to deliver Nurse-Family Partnership services in Monmouth County, New Jersey. You will conduct in-home visits, support first-time mothers, and help families achieve healthier pregnancies and child development.

The role requires an Associate's degree in Nursing, 2–3 years of nursing experience, strong communication, and English/Spanish language skills preferred.

Qualifications

• Associate's degree in Nursing required.
• 2-3 years of relevant nursing experience; homevisiting or maternal/child health preferred.
• Proficiency in computer use and strong verbal/written communication.
• Bilingual in English and Spanish preferred.

Responsibilities

• Conduct in-home visits to provide nursing services aligned with the NFP model and manage a caseload of up to 25 clients.
• Assess and address the health and developmental needs of women and their families.
• Offer counseling and instruction to help families achieve targeted health and life goals.
• Build and maintain strong community relationships to facilitate client referrals and resource connections.
• Attend weekly team meetings, case conferences, and clinical supervision sessions.
• Mentor and precept new hires and support student nurses during home visits as needed.
• Ensure compliance with agency infection control and safety policies.
• Maintain professionalism in adherence to organizational policies on absenteeism and appearance.

As a HomeVisitorRN, you will:

This position is covered by a collective bargaining agreement and is represented by a Union.

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $40.00 to USD $49.00.
