Job summary

Presbyterian Healthcare Services provides, coordinates, and facilitates Home Visitation services with families, physicians, and other organizations through an approved curriculum.

Responsibilities include coordinating care, admitting families, connecting networks, and ensuring contract compliance while maintaining active client engagement.

Qualifications

  • Knowledge of pre-conception to Early childhood development.
  • Function as member of the Early childhood network.
  • Infant Mental Health endorsement within 3 years of hire.
  • Bilingual preferred.

  • Coordinates and facilitates collaborative client care with families and Home visitation staff.
  • Manages evaluations and home visitation services using an approved curriculum.
  • Admits families (prenatal to age three) to the program.
  • Connects families with formal and informal community networks.
  • Fosters multidisciplinary collaboration and decision making with families.
  • Maintains knowledge of available resources for referrals.
  • Ensures compliance with contractual standards and requirements.
  • Data collection and documentation per contract.
  • Maintains active caseload with at least 40% direct family contact.
  • Attends required contractual trainings.
  • Performs other functions as required.

Responsibilities
