Job summary

VNA Health Group is seeking a Full Time Licensed Practical Nurse (LPN) to provide in-home patient care in Middlesex County, New Jersey. You will work under RNs and clinicians to implement care plans, monitor patient progress, and promote quality of life in a home health setting.

The ideal candidate has 2+ years of med/surg or acute care experience, NJ LPN licensure, and strong communication and organizational skills.

Qualifications

• Graduation from an NLN-accredited LPN program with active New Jersey LPN licensure.
• 2+ years of med/surg or acute care nursing experience; home care or hospice a plus.
• Strong verbal and written communication skills.
• Excellent computer skills.
• Exceptional organizational abilities.
• Valid driver’s license and reliable transportation for local travel.

Responsibilities

• Provide Exceptional Care: Implement care plans under the direction of Registered Nurses, ensuring the highest standards of patient care.
• Empower Patients: Assist clients with preventative and rehabilitative nursing procedures to improve their health and well-being.
• Collaborate with a Caring Team: Build strong relationships with patients, families, and healthcare providers to deliver coordinated care.
• Monitor Care Quality: Ensure that services are delivered with attention to quality and appropriateness.
• Follow Safety Protocols: Maintain a safe, clean environment by adhering to infection control and organizational policies.
