Job summary

VNA Health Group in New Jersey seeks a Full Time Licensed Practical Nurse (LPN) to provide compassionate, patient-centered home health care under the supervision of Registered Nurses.

The ideal candidate will have at least 2 years of med/surg or acute care nursing, NJ LPN licensure, strong communication skills, and a valid driver’s license with reliable transportation for local visits.

Qualifications

• Graduation from an NLN-accredited LPN program with active New Jersey LPN licensure.
• 2+ years of med/surg or acute care nursing experience; home care or hospice a plus.
• Strong verbal and written communication skills, excellent computer skills, and organizational abilities.
• Ability to collaborate with patients, families, and healthcare professionals.
• Valid driver’s license and reliable transportation for local travel.

Responsibilities

• Provide Exceptional Care: Implement care plans under the direction of Registered Nurses, ensuring high standards of patient care.
• Empower Patients: Assist clients with preventative and rehabilitative nursing procedures to improve health and well-being.
• Collaborate with a Caring Team: Build strong relationships with patients, families, and healthcare providers to deliver coordinated care.
• Monitor Care Quality: Ensure services are delivered with attention to quality and appropriateness.
