Job summary

Outreach is seeking a Home Visiting Support Specialist II in Luzerne County. The role delivers home-visiting services to 6–8 enrolled families using Parents as Teachers and SafeCare Augmented curricula. It also provides community-based education and supports program managers with daily staff coordination, monitoring, and data tracking.

Hours are 37.5 per week, with some evenings and weekends as needed. A BA/BS in a related field and required certifications are expected, along with strong

Qualifications

• Bachelor's degree in social work, human development, psychology, neuroscience, early childhood education, or a related field preferred.
• Familiarity with working with diverse cultures and families facing economic challenges is a must.
• Mandatory certification training (expenses paid by Outreach).

Responsibilities

• Provide home visits to families with children birth through age five, supporting Parent-Child Interaction, Family Well-Being, Development-Centered Parenting, Health, and Safety.
• Maintains related files/record keeping and completes program-related data entry.
• Plans individual and group activities to stimulate language, social, and motor development in children.
• Provides follow-up to families and maintains appropriate recordkeeping.
• Collaborates with community partners to connect families with resources and referrals.
• Daily support for Parents as Teachers and SafeCare Augmented staff in Luzerne County Office.
