Qualifications

• Bachelor's or MSW in Social Work from an accredited program.
• Licensure: Texas Certification; Texas Driver’s License.
• One year social worker experience in healthcare.
• Reliable transportation and auto liability insurance.

Responsibilities

• Assess hospice patient to identify psychosocial, financial, environmental, and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
