Care at Home is seeking an experienced Social Worker to oversee a range of clinical social work services in inpatient, ambulatory, and community settings. You will conduct psychosocial assessments, develop treatment plans, and collaborate with the care team to support patients and families through illness, hospitalization, and transitions.

The role emphasizes crisis intervention, discharge planning, and coordination with community resources, while ensuring compliant documentation and adherence

Qualificações

• Master’s degree in Social Work from an accredited institution.
• Licensed Independent Clinical Social Worker (LICSW).
• Five (5) years of related experience.
• CPR certification required upon hire if hired before 9/28/2026; if hired after, BLS certification within 30 days.
• Experience in inpatient/mental health settings preferred.

Responsabilidades

• Provide psychosocial assessment of patients and families, including limitations and strengths.
• Conduct crisis interventions for high-risk situations.
• Assist with discharge planning and in-home care arrangements.
• Collaborate with healthcare team to integrate psychosocial factors into care.
• Advise on community resources and referrals; act as patient advocate.
• Maintain clinical documentation per departmental standards and policies.
