The role offers a flexible PRN schedule, a supportive team, and the chance to impact clients’ daily living with mobility improvements and personalized care plans.

Qualificações

• Licensed PT with Home Care experience.
• Strong communication and documentation skills.
• Willingness to travel within the service area up to 1 hour one way.

Responsabilidades

• Provide functional mobility and gait training in clients' homes.
• Deliver post-surgical rehabilitation and recovery support.
• Implement fall prevention strategies and home safety planning.
• Educate clients and families; develop and implement personalized care plans.
