Job summary

LiveWell Partners’ Hope Home Care Network is seeking a licensed Physical Therapist to evaluate patients in home care settings and develop/oversee care plans. The PT delivers therapy services and supervises assistants within a collaborative interdisciplinary team.

The role requires 1 year (2+ preferred) PT experience, active state licensure, CPR certification, and a valid driver’s license. This PRN position offers a per-visit and per-day compensation with mileage reimbursement and flexible

Qualifications

• Minimum of 1 year of physical therapy experience; 2+ years preferred in acute/post-acute settings.
• Active state licensure in physical therapy is required.
• Proof of current CPR certification.
• Valid driver’s license is required.

Responsibilities

• Determine physical therapy treatment plans in consultation with care team and physicians.
• Provide and complete all proper documentation.
• Administer physical therapy treatments as ordered by a physician; supervise assistants.
• Evaluate effects of treatments and prosthetic/orthotic devices; adjust as needed.
• Complete discharge planning and coordinate continuation of therapy post-discharge.
• Participate in weekly case conferences and communicate across disciplines.
