Qualifications
Current Arizona Nurse Practitioner license or PA-C in good standing.
Wound Care and Palliative Care Certificates a plus.
Ability to travel 75+% of the time in the assigned area.
Responsibilities
Provide care to assigned panel of members in their place of residence.
Develop a comprehensive plan of care for patients and their families.
Document member visits and interactions according to protocols.
