Qualifications

• Current Arizona Nurse Practitioner license or PA-C in good standing.
• Wound Care and Palliative Care Certificates a plus.
• Ability to travel 75+% of the time in the assigned area.

Responsibilities

• Provide care to assigned panel of members in their place of residence.
• Develop a comprehensive plan of care for patients and their families.
• Document member visits and interactions according to protocols.
