Abode Care Partners is hiring a Nurse Practitioner to provide primary care services to seniors in home settings across Florida. You will perform clinical assessments, develop personalized care plans, and collaborate with interdisciplinary teams to ensure seamless care transitions.

The role emphasizes preventative care, chronic/acute management, and building meaningful patient relationships in patients’ homes, with opportunities for performance-based incentives and a comprehensive benefits

Qualifications

• Active state NP/PA license and CPR current.
• Current DEA licensure.
• Five+ years nursing healthcare experience; at least two years PA/NP experience preferred.
• RN license in good standing if NP, with current state license.
• Licensed driver with valid automobile insurance.
• Ability to engage in moderate physical activity.

Responsibilities
