Job summary

Premier-Systems-Inc in Indianapolis, IN seeks a Physical Therapy Assistant to provide skilled services in patients' homes under the direction of a licensed Physical Therapist. The role requires travel to visits across west and south Indy with a full-time salary range.

You will assist in evaluation, help develop and monitor a rehabilitation plan, document progress, and educate patients and families on exercise programs. Indiana PTA license and CPR certification are required.

Qualifications

• Graduated from a CAPTE-accredited PTA program.
• Licensed as Physical Therapy Assistant in Indiana.
• Criminal background check completed.
• Current CPR certification.

Responsibilities

• Provide patient services under the direction of a Registered Physical Therapist.
• Assist in evaluation and development of rehabilitative plan of care.
• Prepare clinical notes and progress reports; submit weekly.
• Instruct patients and families in exercise programs.
