Job summary

Blue Delta Tech is hiring a Physical Therapy Assistant to provide skilled PT services in patients' homes in the Indianapolis area, traveling to locations near west and south Indy. This is a full-time role with a salary range of $70k-$75k based on experience, and a part-time per-visit option may be available.

Qualifications

• Graduate of a CAPTE-accredited Physical Therapist Assistant program.
• Licensed in Indiana as a Physical Therapist Assistant.
• Criminal background check required.
• Current CPR certification required.

Responsibilities

• Provide patient services under the direction of a licensed Physical Therapist.
• Assist in evaluation and development of the rehabilitative plan of care and re-evaluation as needed.
• Prepare daily progress notes and submit weekly summary reports.
• Assist patients with routine strengthening exercises and use of self-help devices.
• Instruct patients and families in home exercise programs and proper technique.
• Document outcomes and report changes to the PT and physician.
• Collaborate with health team and participate in discharge planning.
• Support home visits scheduling and coordination with nursing supervisors.
