Job summary

Century Impact Orthopedic Physical Therapy seeks a dedicated Physical Therapist for full-time, on-site home-based care in Freehold, New Jersey. You will deliver in-home therapy, assess function, and tailor plans to improve mobility and independence.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited program.
• Current NJ physical therapy license in good standing.
• CPR certification required for all staff.

Responsibilities

• Provide home-based physical therapy per the prescribed plan.
• Supervise and instruct physical therapy assistants.
• Conduct comprehensive client evaluations and document findings.
• Assess home environment to tailor rehabilitation plans.
• Deliver PT services in clients' homes using manual and prescribed modalities.
• Document clinical records in compliance with regulations (e.g., OASIS).
• Participate in interdisciplinary team meetings and educate families.
