Work Environment

This role is primarily onsite, focused on providing home care directly to clients in the comfort of their residences. You will be working as part of a collaborative and interdisciplinary team that includes healthcare professionals dedicated to improving patient outcomes. The setting allows for a personalized approach to therapy, enabling you to directly observe the home environment and tailor rehabilitation programs effectively.

Key Responsibilities

• Direct physical therapy treatments aimed at preventing disability, relieving pain, improving muscle function, and maximizing clients’ performance capabilities as prescribed in the treatment plan.
• Supervise and instruct physical therapy assistants to ensure the correct execution of care plans, providing guidance as needed.
• Conduct comprehensive client evaluations using diagnostic tests for muscle, nerve, joint, and functional abilities to assess levels of function.
• Analyze the home settings of clients to identify potential barriers to achieving independence and adapt care plans accordingly.
• Deliver prescribed physical therapy services in the client's home, administering manual and non-manual therapeutic exercises as well as traction and massage techniques.
• Evaluate, fit, and adjust prosthetic and orthotic devices, recommending modifications to the supervising physician when necessary.
• Document detailed clinical records, including initial evaluations, care plans, progress notes, and discharge summaries compliant with regulations like OASIS.
• Participate actively in interdisciplinary team meetings to develop and reassess treatment plans, ensuring a comprehensive approach to client care.
• Educate clients and their families on the use of therapeutic devices, as well as overall physical therapy programs.
• Be available to provide support and oversight to Physical Therapy Assistants during client treatment sessions.
• Complete administrative duties, such as timely submission of clinical and payroll documentation according to company policies.
• Engage in ongoing professional development and contribute to the company's mission of clinical excellence and exceptional service delivery.
