Premier Health Partners is seeking a Physical Therapist/Home Care Case Manager to evaluate and provide physical therapy services in the Greater Cincinnati, Ohio area. The role includes coordination of all components of a patient’s home care services.

The ideal candidate will have a Bachelor's degree in Physical Therapy, current Ohio licensure, CPR Certification, and 1-3 years of clinical experience. Strong skills in adaptability and self-direction are also preferred.

Qualifications

• Graduate of an accredited Physical Therapy Program.
• Current licensure to practice Physical Therapy in Ohio.
• CPR Certification.

Responsibilities

• Evaluate and supervise provision of physical therapy services.
• Coordinate all components of a patient's home care services.
• Facilitate development and implementation of care plans.
