Job summary

Capital Health Care Network in Miamisburg, OH seeks a licensed physical therapist to provide PT services per physician's plan of care, assess status, evaluate mobility, and help lead rehabilitation programs for patients across home health settings.

Qualifications

• PT degree from an accredited program.
• Licensed to practice as a physical therapist in Ohio.
• Two years of physical therapy experience; home health preferred.
• Demonstrates good verbal and written communication and organization skills.
• Current CPR certification.

Responsibilities

• Provides physical therapy services to patients per physician's plan of care.
• Assesses therapeutic/rehabilitative status and contributes to the total plan of care.
• Evaluates muscle strength, mobility, gait, ROM for rehab potential.
• Directs physical therapy treatment and instructs patients and families.
• Initiates PT program and educates staff and caregivers.
• Participates in discharge planning and submits progress summaries.
• Provides PT consultation and in-service education as needed.
• Supervises homemaker/home health aides when assisting in rehabilitation.
