Job summary

Ohio Living is seeking a licensed Physical Therapist to perform evaluations and provide treatments in accordance with physician orders and state guidelines. Qualifications

• Must be able to read, write, speak, and understand English.
• Experience with geriatric and physical therapy patients.
• Knowledge of treatment modalities.
• Current unencumbered Ohio license to practice as a Physical Therapist (PT).

Responsibilities

• Initiates contact with physician for orders and treatment plan.
• Performs PT treatments and evaluates patient progress.
• Educates patients and family on exercises and safety.
• Participates in discharge planning and interdisciplinary team meetings.
• Maintains patient records in EMR and paper charts.
