Job summary

Innovive Health in Massachusetts seeks a licensed Physical Therapist to provide home-based PT services. You will perform evaluations, start of care, follow-ups, and discharges in patients' homes under physician orders.

You'll educate patients on safe home exercises, gait training, fall prevention, and use of DME, while documenting progress and coordinating with the care team. Home health experience is preferred and CPR certification is required.

Qualifications

• Must hold a valid Massachusetts physical therapy license.
• Home care experience required.
• CPR certification current.
• Valid driver’s license.

Responsibilities

• Conducts home physical therapy visits and evaluations.
• Develops and implements treatment plans with physician input.
• Educates patients on home exercise, safety, and fall prevention.
• Documents progress, adjusts rehab plan as needed.
• Coordinates discharge planning with care team.
• Maintains infection control and patient confidentiality.
• Provides DME guidance and orders when appropriate.
