Job summary

Qualifications

• Must have a degree in Physical Therapy from an accredited university.
• Valid license to practice Physical Therapy within the state of Massachusetts.
• Current CPR certification.
• Valid driver’s license.
• Home health care experience is required.

Responsibilities

• Perform home physical therapy visits, evaluations, start of care, follow ups, and discharges.
• Develop and modify treatment plans in consultation with physicians.
• Educate patients and caregivers on exercises, safety, and fall prevention in the home.
• Assess vitals and monitor patient safety, reporting abnormal findings.
• Document patient progress and adjust plans as needed.
• Coordinate discharge planning with care team and physicians.
