Job summary

Pivotal Health Direct Care is seeking a full-time nurse practitioner to join a mobile home-visit care model in Northeast Wisconsin. You’ll deliver friendly, compassionate, and convenient care in patients’ homes using a company-provided vehicle, handling general and preventive care across all ages and minor procedures.

You’ll coordinate specialist referrals and primary care follow-ups with a collaborative clinical team, require Wisconsin NP licensure, board certification, DEA license,

Qualifications

3+ years NP experience in EM/Urgent Care/Family Practice.
Unrestricted Wisconsin NP license and national board certification.
DEA license and prescriptive authority.
Valid driver’s license with a clean driving record.

Responsibilities

Provide care in patients’ homes using a company-provided vehicle.
Deliver general and preventive care for all ages.
Perform minor procedures such as splinting, wound care and suturing.
Specialist referrals and primary care follow-ups.
