Job summary

Pivotal Health Direct Care is seeking a full-time nurse practitioner to join a mobile home-visit care model in Northeast Wisconsin. You’ll deliver friendly, compassionate, and convenient care in patients’ homes using a company-provided vehicle, handling general and preventive care across all ages and minor procedures.

You’ll coordinate specialist referrals and primary care follow-ups with a collaborative clinical team, require Wisconsin NP licensure, board certification, DEA license,

• Unrestricted Wisconsin NP license and national board certification.

Your Role

As a full-time nurse practitioner, you’ll be the face of Pivotal Health, delivering:

• Friendly, compassionate, and convenient care in patients’ homes using a company-provided vehicle equipped with necessary supplies and tools.
• General and preventative care for all ages.
• Minor procedures (splinting, wound care, suturing).
• Specialist referrals and primary care follow-ups.

Minimum Qualifications:

• At least 3 years of nurse practitioner experience in Emergency Medicine, Urgent Care, or Family Practice.
• Unrestricted Wisconsin nurse practitioner license and national board certification.
• DEA license and prescriptive authority.
• A valid driver’s license with a clean driving record.

Skills and Competencies:

• Expertise in acute, chronic, and preventive care across diverse populations.
• Proficiency with electronic medical records (EMR).
• Strong interpersonal and communication skills.
• Confidence in performing therapeutic interventions such as splinting and suturing.
