Job summary

Southcentral Foundation (SCF) in Anchorage is seeking a Nurse Practitioner - Specialty Home Visit Team to deliver comprehensive, home-based medical care for individuals with complex needs within the Anchorage service area.

The Home Visit Provider works with an interdisciplinary team to promote continuity of care and wellness, requiring Alaska NP licensure, AANP/ANCC certification, and BLS (with ACLS/NRP/ATLS as needed).

Qualifications

• Licensed as a Nurse Practitioner in the State of Alaska.
• Current certification by the American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
• Meets all requirements to obtain associate medical staff membership and required privileges of service.
• Basic Life Support (BLS) certification is required. ACLS, NRP, ATLS, ALSO and PALS as required for specialty and any certification relating to the scope of practice as required.

Responsibilities

• Deliver comprehensive, home-based medical care to individuals with complex medical needs on the Home Visit Team.
• Collaborate with an interdisciplinary team to promote continuity of care, independence, and overall wellness.
