Job summary

Northeast Healthcare Recruitment, Inc. is seeking Nurse Practitioners and Physician Assistants to join our home-based patient care team in Georgia.

You will conduct initial screenings, create assessment plans, and coordinate urgent and follow-up visits for patients in their homes. Responsibilities include performing exams, ordering tests, managing medications (including psychopharmacology), delivering palliative and end-of-life care, and ensuring accurate documentation.

Qualifications

• Valid NP or PA license and board certification.
• Active NPI number.
• BLS/ACLS certification.
• Valid driver's license and auto insurance.
• Willingness to drive to patients' homes.

Responsibilities

• Perform initial patient screenings and physical examinations.
• Create assessment plans and coordinate urgent and follow-up visits.
• Order, interpret, and analyze diagnostic tests.
• Prescribe and manage medications and treatments as appropriate.
• Deliver palliative and end-of-life care when needed.
• Maintain accurate patient records and documentation.
