Qualifications

• Experience in family practice, geriatrics, hospice, palliative care, hospitalist, ICU, SICU, ER.
• Valid NP license / Board Certification.
• NPI as an NP.
• BLS/ACLS.
• Valid driver's license and auto insurance; willingness to drive to patients' homes.

Responsibilities
