Job summary

PruittHealth in Georgia-Augusta seeks an experienced Nurse Practitioner or Physician Assistant to provide holistic, home-based care and telehealth health risk assessments for members of the Premier Plan. You will work with physicians and care teams to diagnose, manage, and prevent illness, while coordinating transitions across care settings and participating in QAPI initiatives.

Qualifications

• Need Master’s degree from an accredited Adult/Family Nurse Practitioner program or PA Studies.
• Board certification in Adult, Family or Gerontology care.
• 2+ years of clinical NP/PA experience in geriatric/adult or family practice.

Responsibilities

• Perform comprehensive adult/geriatric admission history and physical exams.
• Identify, diagnose and document episodic, acute and chronic diseases in adults.
• Coordinate care during transitions across levels of care and settings.
• Reconcile medications and assess medication-related problems.
• Collaborate with physicians, case managers, social workers and families.
• Engage in Quality Assurance and Performance Improvement (QAPI) activities.
