Job summary

Ascend Health is hiring a Licensed Practical Nurse for home care in New Jersey. The LPN will communicate clearly, maintain patient confidentiality, and work collaboratively with the health care team to provide quality nursing care.

Responsibilities include performing nursing assessments under RN supervision, documenting care, and ensuring patient safety and comfort in the home setting. A NJ LPN license and at least 1 year of community health experience are required.

Qualifications

• High School Diploma or equivalent.
• Current LPN license within the State of New Jersey.
• 1 year of Community Health Nursing.
