Job summary

Atlantic Health System is seeking a Licensed Practical Nurse to join our team for home visits and ambulatory support. The role involves delivering care per plan, documenting promptly, and collaborating with RNs to ensure patient safety.

The LPN must have a valid NJ license and at least one year of acute, subacute, or home care experience, with proficiency in EMR systems and clear communication with patients and families. Travel to patient homes is required.

Qualifications

• Current NJ LPN license.
• Minimum of one-year experience in acute, subacute, or home care setting.
• Experience using an EMR and excellent communication skills.

Responsibilities

• Delivers safe care according to plan of care and as delegated by the RN.
• Documents care within time frames.
• Collaborates with health care team.
• Reports all findings to RN or designee.
• Notifies MD of significant changes.
• Assists RN in patient/family communication.
• Provides written teaching material to support learning.
• Documents all communication timely.
• Follows infection control practices.
• Cross trains to learn new skills.
• Use of personal vehicle is required for home visits. Driver's License verification required.
