Moms In Motion is seeking a Service Facilitator to visit clients in their homes across Virginia, guiding families through Medicaid Waiver programs and creating care plans to support independent living.

The role emphasizes compassionate communication, organizational skills, and tech-savviness for digital records and video calls. A valid driver’s license and background check are required, with remote work optional from a laptop/tablet.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; relevant heart and experience valued.
• Tech-savvy enough for email, docs, spreadsheets, portal navigation and video calls.
• Must pass a background check and provide 2 professional references.
• Bonus points for Person-Centered Thinking/Planning training.

Responsibilities

• Hit the road locally to visit clients in their homes.
• Be the go-to guide for families navigating waiver programs.
• Write up plans of care and assessments that actually make a difference.
• Troubleshoot challenges like service authorizations and timesheets.
• Build lasting relationships with families based on respect, patience, and trust.
• Work remotely from your laptop/tablet.
