The State College Area School District is seeking a Home School Visitor Long Term Substitute to support student well-being and equitable access to resources. The HSV liaises with families, staff, and community partners within the Student Services Department and begins February 24, 2027, with dates subject to change.

Responsibilities include attendance oversight, coordinated meetings, home visits, and collaboration with the District Social Worker to ensure services align with district policies

Qualifications

• Bachelor degree preferred.
• Citizenship, residency or work visa required.
• PA Home and School Visitor Certification required or able to obtain.

Responsabilités

• Attendance Oversight: monitors attendance procedures, truancy assessments, SAIP meetings, and serves as court liaison.
• Student Support & Advocacy: participates in SAP/MTSS/IEP meetings, conducts home visits, connects families to resources.
• Basic Needs Coordination: links families to services and supports the Needs Assessment process.
• Homeless & Foster Care Liaison: acts as point of contact and coordinates support plans.
• Community Engagement: represents the district on local committees and collaborates with agencies.
• Professional Collaboration: partners with School Social Worker and staff to address barriers.

Job Description

The State College Area School District is seeking a long-term substitute Home and School Visitor. The Home and School Visitor serves as a vital link between students, families, and school personnel within the State College Area School District. As part of the Student Services Department, the HSV fosters strong home-school partnerships to support student well-being, academic success, and equitable access to resources. This temporary assignment will tentatively begin February 24, 2027, with dates subject to change based on operational needs.

Primary Responsibilities

• Attendance Oversight: Implements and monitors attendance procedures, conducts truancy assessments, coordinates SAIP meetings, and serves as court liaison for truancy cases.
• Student Support & Advocacy: Participates in SAP, MTSS, and IEP meetings; conducts home visits; facilitates access to school and community resources; and supports restorative practices.
• Basic Needs Coordination: Connects families to services (food, clothing, housing, mental health), manages the Helping Hand Pantry, and supports students through the Needs Assessment process.
• Homeless & Foster Care Liaison: Serves as point of contact, monitors student needs, and coordinates support plans for displaced families.
• Community Engagement: Represents the district on various local committees (e.g., CAC, CTC, CCCHS), collaborates with agencies, and promotes social service initiatives.
• Professional Collaboration: Partners with the District Social Worker, school staff, and external providers to address barriers to student success and ensure compliance with laws and policies.
