Job summary

Mosaic in Colorado is seeking Home Providers to welcome individuals with intellectual disabilities into their homes and support them toward independence, growth, and belonging. You’ll become part of the household and family dynamics, matching based on interests and support needs to help both thrive.

Responsibilities include daily living skills, medication assistance, community participation, transportation, and documenting progress with Mosaic's support team.

Qualifications

• Must be at least 18 years old.
• Current resident of Colorado (due to state funding restrictions).
• Own or rent your home, apartment, condo, or townhouse.
• Active homeowner's or renter's insurance.
• Reliable transportation, valid driver's license, and current vehicle insurance.
• Internet access and basic computer skills.
• Ability to complete required screenings, background checks, and training.
• Experience is preferred but not required.

Responsibilities

• Supporting daily living skills such as cooking, budgeting, cleaning, and decision-making.
• Assisting with medication administration and medical appointments.
• Helping someone build connections, participate in their community, and experience a sense of belonging.
• Providing transportation and support with daily activities as needed.
• Maintaining documentation and communication with the Mosaic support team.
• Helping someone build confidence, develop independence, and pursue personal goals.
