Job summary

Enliven is seeking an experienced Registered Nurse (RN) to service home infusion patients in Phoenix and surrounding locations. The nurse will educate patients and families on prevention and healthy habits, follow health guidelines, and handle emergent situations with little supervision.

Qualifications

• Associate or Bachelor Degree in Nursing with active RN license.
• Minimum five years of nursing experience; acute care preferred.
• Experience with home health or infusion is a plus.
• Strong clinical judgment and patient education skills.

Responsibilities

• Complete daily EMR forms for each patient.
• Conduct admission, case management and follow-up visits.
• Administer ongoing care and infusions per physician orders.
• Collaborate with physicians and pharmacists on treatment plans.
• Educate patients and families on prevention and healthy habits.
• Document care plans and discharge summaries.
• Report patient progress to the physician, clinical manager, and team.
• Prepare visit reports and coordinate with care teams.
