Enliven Home Infusion Specialists Colorado is seeking an experienced RN to service home infusion and infusion center patients in Longmont and surrounding areas. You will educate patients and families on prevention and healthy habits while delivering high‑quality nursing care with minimal supervision.

The ideal candidate has strong Med/Surg/ICU experience, current CPR/BLS, and ACLS/PALS preferred; home health/infusion exposure is a plus. Flexible schedule and travel reimbursement are offered.

Aufgaben

• Complete daily electronic forms for each assigned patient within a secure EMR system.
• Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.
• Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager.
• Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment.
• Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation.
• Report patient care, conditions, and progress to the patient's physician, pharmacy, and Clinical Manager continuously.
• Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
• Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
• Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
• Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care.

The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patients' well-being by providing high-quality nursing care.

Responsibilities: RN (Registered Nurse)

Job Requirements: RN (Registered Nurse)

• Associate or Bachelor Degree in Nursing Minimum of five years of nursing experience preferred in an Acute Care setting
• Strong Med/Surg, ICU, ER, acute experience
• Home Health/Infusion experience (not required, but preferred)
• Current CPR/Basic Life Support certifications (BLS) required
• Advanced Cardiac Life Support (ACLS); Pediatric Advanced Life Support (PALS) (preferred)
• Skilled in accessing Implanted Ports; Central Lines; PICC lines
• Strong IV placement skills
• Strong organizational and communication skills
• Preferably COVID-19 Vaccinated (some pharmacy partners & patients require a vaccinated nurse)
