Job summary

Enliven Home Infusion Specialists Arizona is seeking an experienced RN to service home infusion and infusion center patients in city name and surrounding locations. The RN will educate patients and families on prevention and healthy habits, and will follow health and safety guidelines to promote patient well-being.

Responsibilities include admission, case management, and follow-up nursing visits, care administration for infusion patients, and collaboration with physicians and pharmacies to craft

Qualifications

• RN with at least five years of nursing experience, preferably in acute care.
• Strong Med/Surg, ICU, ER experience.
• Home Health/Infusion experience preferred.
• Current CPR/BLS required; ACLS and PALS preferred.
• Skilled with ports, central lines, PICC and IV placement.
• Excellent organizational and communication skills.
• COVID-19 vaccination preferred by some partners.
• Background check, malpractice insurance, and drug screen required.

Responsibilities

• Complete daily electronic forms for each assigned patient within a secured EMR system.
• Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.
• Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager.
• Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment.
• Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation.
• Report patient care, conditions, and progress to the patient's physician, pharmacy, and Clinical Manager continuously.
• Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
• Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
• Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
• Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care.

The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patient's well-being by providing high-quality nursing care.

• Associate or Bachelor Degree in Nursing Minimum of five years of nursing experience preferred in an Acute Care setting
• Strong Med/Surg, ICU, ER, acute experience
• Home Health/Infusion experience (not required, but preferred)
• Current CPR/Basic Life Support certifications (BLS) required
• Advanced Cardiac Life Support (ACLS); Pediatric Advanced Life Support (PALS) (not required, but preferred)
• Skilled in accessing Implanted Ports; Central Lines; PICC lines
• Strong IV placement skills
• Strong organizational and communication skills
• Preferably COVID-19 Vaccinated (some pharmacy partners & patients require a vaccinated nurse)
