Job summary

Enliven is seeking an experienced RN to service home infusion and infusion center patients in Denver, CO and surrounding locations. The ideal candidate must be well-trained and able to deliver high-quality nursing care with minimal supervision and strong patient education.

You will complete daily EMR entries, administer infusions, collaborate with physicians and pharmacies, and ensure accurate care plans and documentation.

Qualifications

• Associate or Bachelor Degree in Nursing is required.
• Minimum of five years of nursing experience preferred in an Acute Care setting.
• Current CPR/BLS certification required.
• ACLS; PALS preferred.
• Skilled in accessing Implanted Ports; Central Lines; PICC lines.
• Strong IV placement skills.
• COVID-19 vaccination preferred by some partners/patients.
• Background check, Malpractice insurance, and Drug screen required.

Responsibilities

• Complete daily EMR entries for each assigned patient within a secure EMR system.
• Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.
• Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager.
• Confer with physicians and pharmacies to develop the initial treatment plan based on orders and initial assessment.
• Provide hands-on care, management, and evaluation of the care plan under physician supervision.
• Report patient care, conditions, and progress to the physician, pharmacy, and Clinical Manager continuously.
• Implement patient care plan with patient and family to achieve optimal resolution of needs.
• Discharge patients after consultation with physician and Clinical Manager, with proper documentation.
• Prepare medical documentation including case conferences, patient contacts, and care plan changes.
• Prepare visit/shift reports and update patient records; coordinate with other healthcare disciplines.

The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patients' well-being by providing high-quality nursing care.

Responsibilities: RN (Registered Nurse)

• Complete daily electronic forms for each assigned patient within a secure Electronic Medical Record (EMR) System.
• Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.
• Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager.
• Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment.
• Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation.
• Report patient care, conditions, and progress to the patient's physician, pharmacy, and Clinical Manager continuously.
• Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
• Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
• Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
• Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care.

• Strong Med/Surg, ICU, ER, acute experience
• Home Health/Infusion experience (not required, but preferred)
• Current CPR/Basic Life Support certifications (BLS) required
• Advanced Cardiac Life Support (ACLS); Pediatric Advanced Life Support (PALS) (preferred)
• Skilled in accessing Implanted Ports; Central Lines; PICC lines
• Strong IV placement skills
• Strong organizational and communication skills
• Preferably COVID-19 Vaccinated (some pharmacy partners & patients require a vaccinated nurse)
