Job summary

The PromptCare Companies is seeking a skilled Home Infusion Pharmacist to join our team in Cedar Rapids, IA on a per diem basis. You will review charts and lab results for an assigned caseload of patients receiving specialized infusion therapy and collaborate with physicians and nurses to ensure high-quality care.

Responsibilities include evaluating medication orders, monitoring patient progress, supervising staff in sterile compounding, and contributing to training initiatives.

Qualifications

• B.S. or PharmD required; registered pharmacist license if foreign graduate.
• Active license in the state with any required multi-state privileges.
• 1 year hospital and home infusion therapy experience preferred.
• 2 years of recent clinical pharmacy experience preferred.
• Thorough knowledge of infusion therapies preferred.
• Recent inventory management experience preferred.

Responsibilities

• Participate in patient evaluation and chart review for acceptance, product selection, device selection and supply management.
• Collect, interpret, and evaluate physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness.
• Monitor patient progress and communicate clinical information with physician and healthcare team.
• Directly supervise pharmacy technicians in compounding, packaging, and labeling of sterile products.
• Assist in training and orientation of new employees and performance improvement activities.
• May require licensure in multi-state service areas.
• Go into IV room to mix medications if needed.
• Other duties as assigned.
