Job summary

Option Care Health seeks a Clinical Pharmacist to provide safe pharmacy services for home infusion patients, coordinating with nursing and education teams. This entry-level role focuses on patient-centered pharmacotherapy and education, under policy and regulatory guidelines.

You will ensure accurate dispensing, device calibration, and safety, while maintaining compliance with DEA, USP, and state laws. Growth opportunities and professional CEU participation are encouraged.

Qualifications

• Pharmacist graduate with active license and state registration.
• Experience or training in sterile IV, MTM or residency preferred.
• Strong communication and documentation skills required.

Responsibilities

• Assess patient suitability for home care per policy and prognosis.
• Design and monitor patient-specific pharmacotherapy plans.
• Educate patients, caregivers and staff on medications.
• Ensure continuity of care between home and other settings.
• Prepare and dispense medications with proper labeling and safety.
