KabaFusion is seeking a licensed Pharmacist to provide safe, compliant pharmacy services in the home infusion space. You will assess therapy suitability, dispense medications, document patient progress, and coordinate with clinicians to optimize outcomes.

Qualifications

• Bachelor of Science in Pharmacy and active state license.
• Minimum two years acute-care or one year home infusion experience.
• Knowledge of intravenous admixture services and therapy.

Responsibilities

• Adhere to ethical pharmacy practices and regulations.
• Assess suitability of home care and prescribed therapy.
• Design patient-specific plans and document progress.
• Prepare and dispense medications and supplies cost-effectively.
• Document communications with physicians, nurses, and patients.
• Maintain patient medication profiles and dosing accuracy.
• Ensure QA/QC and regulatory compliance in compounding.
