Job summary

KabaFusion in Louisville, KY is seeking a pharmacist to join a mission-driven home infusion team focused on patient care and positive clinical outcomes.

As a pharmacist with us, you will collaborate with providers, communicate with patients, and perform clinical review as an integral part of the continuum of care.

Requirements include a PharmD or BS in Pharmacy, a current state license, and 1–2 years of IV admixture experience; we offer generous referral programs.

Qualifications

• Pharmacist degree from an accredited college; PharmD preferred.
• Current license as a registered pharmacist in the state of employment.
• 1–2 years of IV admixture/IV therapy experience.

Responsibilities

• Demonstrates ethical pharmacy practice and ensures compliance with all company policies and regulations.
• Assesses laboratory results and performs pharmacokinetic dosing.
• Performs accurate prescription order entry to ensure medications are properly prepared and stored.
